TETHRLINE — PERMANENT JEWELRY CONSENT, ASSUMPTION OF RISK & RELEASE OF LIABILITY

By signing this agreement, I voluntarily consent to receive permanent jewelry services from TETHRLINE.

UNDERSTANDING THE SERVICE

I understand that permanent jewelry involves custom-fitting a chain and welding its ends together for a clasp-free finish. The chain is welded closed, not attached to my skin. Although intended for continuous wear, the jewelry can be removed by cutting the chain and may require removal for medical procedures, work, sports, or other activities. “Permanent” does not mean indestructible or guaranteed to remain attached.

ACKNOWLEDGMENT OF RISKS

I understand that installation and wear involve risks, including heat exposure, burns, eye injury, skin irritation, allergic reactions, cuts, swelling, circulation restriction, and injuries caused by jewelry snagging, catching, tightening, or being pulled. Entanglement with machinery or other objects can cause severe injury. In severe circumstances, these risks may result in permanent disability or death. I understand that continuous wear may be inappropriate for certain activities, occupations, or medical circumstances. I agree to remove the jewelry whenever it presents a safety risk and to seek appropriate medical assistance if an injury or reaction occurs.

PERSONAL DISCLOSURES AND SAFETY

Before service, I will inform the technician of known metal allergies, skin sensitivities, implanted medical devices, or other medical concerns relevant to welding or continuous jewelry wear. If I am uncertain whether the service is appropriate for me, I will seek guidance from a qualified healthcare professional before proceeding. I agree to follow the technician’s instructions, remain still during welding, and use the protective equipment provided. I understand that TETHRLINE may decline or stop service for safety reasons.

FIT, CARE AND REMOVAL

I will review and approve the chain’s fit before welding and notify the technician of any discomfort or concerns. I acknowledge that I have received or been given access to care and removal instructions and have had an opportunity to ask questions. I understand that jewelry may break, stretch, tarnish, or become damaged through wear or exposure. I will not rely on the chain breaking automatically if it becomes caught. This agreement does not cancel any express written warranty provided by TETHRLINE.

GUARANTEE

Every connection deserves care. If your weld comes apart within 90 days of purchase due to a workmanship issue, we’ll re-weld your original TETHRLINE chain at no charge. Simply keep your chain and contact us to arrange a repair. The original chain must be fully intact, apart from the separated weld, and brought with you for re-welding. This guarantee covers the weld and excludes chain breakage, stretching, accidental damage, and lost jewelry. Re-welds outside this guarantee are $20.

ASSUMPTION OF RISK AND RELEASE OF LIABILITY

I knowingly and voluntarily assume the risks described above associated with receiving permanent jewelry services and wearing the jewelry. To the fullest extent permitted by New York law, I release and agree not to sue TETHRLINE, its owners, employees, and agents for claims arising from the fitting, welding, installation, wear, or removal of my permanent jewelry, including claims for personal injury, disability, death, or property damage caused by the ordinary negligence of TETHRLINE, its owners, employees, or agents. This release does not apply to gross negligence, reckless conduct, intentional misconduct, or any liability or rights that cannot legally be waived. It does not release claims belonging to other people that I have no legal authority to waive.

FINAL ACKNOWLEDGMENT

I confirm that I am at least 18 years old, am signing voluntarily, and am able to understand this agreement. I have read it, understand that it includes a release of claims involving ordinary negligence, and have had the opportunity to ask questions before receiving service.
